The key criteria for success as a Lead Caregiver are reliability, responsiveness, and versatility. You should be comfortable stepping into a client's home without an introduction, assessing the plan of care, and understanding intuitively what the client’s needs will be. The Lead Caregiver must be available on weekends and evenings. The Lead Caregiver must have Supervisory experience and have the ability to get along with people well, motivate, engage both client and staff, and have excellent organizational skills. Must know how to take vitals and be able to assess client’s health status and home care needs.

In addition to regular shift with a client, the lead caregiver will be given some regularly scheduled hours during the week to assist with administrative overview as part of the Care Support Team. The administrative overview tasks will involve the following:
• Initial client assessment
• Ongoing home visits with clients and caregivers
• Must be able to communicate effectively with both clients and their families as well as the staff, about care of client and identify needs and resolve issues.
• Ensure caregivers are following reporting protocol and guidance provided for each client - based on client’s health status.
• Assist with HHC Applications (New and renewals) for CINICO submission.
• Claim submissions to insurance provider.
• Maintain Caregivers time.

Must be an experienced Certified Nursing Assistant, Home Care Aide or equivalent with excellent references. Prefer someone who can be nurse delegated. A valid driver’s license a must. Although this is a position with guaranteed hours per week the service delivery times are variable, requiring maximum flexibility in scheduling and availability.
